UPHCA Continuing Education Course Enrolment Form

To Register for UPHCA Continuing Education Courses:

  1. Click on the 2011 Continuing Education Applicationlink
  2. Select the course you want and the number of people attending
  3. Calculate the amounts
  4. Complete information form and e-mail to jped@uphca.org  or fax to (801) 255-2923
  5. Make Payments by either going to the “Make a Payment” page and pay by Credit Card OR Send application and check to UPHCA – 8680 Monroe Street, Ste A, Sandy Utah 84070

Courses are on a first come basis with preference to UPHCA members.  72- hour cancellation, thereafter, no refunds can be given, however, credit to other courses may be applied.  Go to the page and
