UPHCA Continuing Education Course Enrolment Form
To Register for UPHCA Continuing Education Courses:
- Click on the “2011 Continuing Education Application” link
- Select the course you want and the number of people attending
- Calculate the amounts
- Complete information form and e-mail to jped@uphca.org or fax to (801) 255-2923
- Make Payments by either going to the “Make a Payment” page and pay by Credit Card OR Send application and check to UPHCA – 8680 Monroe Street, Ste A, Sandy Utah 84070
Courses are on a first come basis with preference to UPHCA members. 72- hour cancellation, thereafter, no refunds can be given, however, credit to other courses may be applied. Go to the page and
